The objective is to determine whether the proposed team understands the risks and evidence requirements of your project, rather than whether the CRO has worked with any medical device.
Evidence to request: anonymised project summaries, device classes, countries, study phases, regulatory pathways, services performed and the roles of the proposed team members.
Warning sign: experience is described only through broad terms such as MedTech, healthcare or clinical trials, without identifying comparable device investigations.
2. Regulatory Pathway Experience
The CRO should be able to explain how the proposed investigation fits within the relevant regulatory framework. For an EU study, this may include MDR Articles 62, 74 or 82, national requirements and the applicable competent authority and ethics committee procedures. For a US study, the assessment should cover the applicable IDE and IRB pathway.
Evidence to request: recent submission experience, examples of authority questions addressed, country-specific submission plans and a proposed responsibility matrix.
Warning sign: the regulatory strategy is presented as globally standardised without recognising national submission differences or the regulatory status of the device.
3. ISO 14155 Processes and Quality System
Do not assess ISO 14155 through a compliance statement alone. Review how the CRO applies the standard to the activities it will perform. ISO 14155:2026 covers the design, conduct, recording and reporting of medical device clinical investigations and defines responsibilities intended to protect subjects and ensure credible results.

4. Proposed Team and Governance
Qualification should cover the people who will perform the work, not only the company’s senior experts. Confirm the named project manager, clinical leads, monitors, data managers, statistician and medical writer, together with their availability and relevant experience.
Evidence to request: CVs, role descriptions, allocation assumptions, training records, backup arrangements, escalation routes and expected staff turnover controls.
Warning sign: senior experts participate in the sales process, but the delivery team is unnamed or will only be assigned after contract signature.
5. Site Feasibility and Monitoring
A site network is useful only when it is relevant to the target population, device, procedure and recruitment assumptions. Review how the CRO selects investigators, validates feasibility data and determines the monitoring model.
Evidence to request: feasibility methodology, investigator qualification criteria, recruitment assumptions, site activation process, monitoring approach, escalation thresholds and examples of monitoring documentation.
Warning sign: recruitment commitments are based on an unverified database or historical relationships rather than current site-level feasibility.
6. Data Management and Biostatistics
Confirm how the CRO will protect data integrity from database design through database lock and final analysis. The assessment should cover systems, validation, access controls, audit trails, data cleaning, coding, statistical planning, privacy and data transfers.
Evidence to request: data management plan template, statistical analysis plan template, system validation information, edit-check process, database-lock procedure, security controls and the list of subcontracted data services.
Warning sign: critical data or statistical activities are outsourced without a documented oversight and quality-control process.
7. Safety Reporting and Vendor Oversight
The CRO should demonstrate how serious adverse events, device deficiencies and other reportable information move from the site to the sponsor and competent authorities. Responsibilities, timelines and reconciliation procedures should be defined before the first subject is enrolled.
Evidence to request: safety management plan, reporting workflow, escalation process, reconciliation procedure, vendor qualification records and oversight arrangements for laboratories, imaging providers or other subcontractors.
Warning sign: the proposal states that safety reporting is included but does not define responsibilities, recipients, timelines or the systems used.
8. Commercial and Operational Fit
Compare proposals using the same scope, assumptions and acceptance criteria. A lower initial fee may exclude activities that later appear as change orders or remain with the sponsor.
Evidence to request: detailed scope, exclusions, deliverables, timeline assumptions, change-order rules, communication cadence, KPIs, audit rights, subcontractors, data ownership and study-transfer arrangements.
Warning sign: the proposal contains an attractive fixed price but does not define volumes, responsibilities, dependencies or the conditions that trigger additional fees.
A Four-Step CRO Qualification Process
- Define the Scope and Evaluation Criteria
List every activity that may be delegated and identify the critical requirements for the device, investigation, countries, data and regulatory pathway. Send the same scope and questions to every shortlisted CRO so that proposals remain comparable.
- Complete the Document Review
Review the proposed team, relevant project experience, SOP index, training records, quality-system evidence, system controls, vendor arrangements, insurance and the assumptions included in the proposal. Record missing or conditional evidence rather than accepting verbal confirmation.
- Conduct a Capability Assessment or Audit
Ask the CRO to walk through a realistic project scenario. Test how it would manage a protocol deviation, urgent safety event, recruitment issue, database change, underperforming site or subcontractor failure. Where the delegated activities are critical, conduct a remote or on-site qualification audit.
- Score the Evidence and Transfer Controls Into the Contract
Score each provider against predefined criteria and document the final decision. The contract, scope of work and responsibility matrix should then reflect the commitments made during qualification, including deliverables, oversight, escalation, audit rights, subcontractors, data ownership and exit arrangements.
Qualification does not end at contract signature. The sponsor should reassess the CRO when the scope changes, critical personnel or vendors change, serious quality issues arise or performance indicates that additional oversight is required.
